Medication

Back pain is a prevalent and often debilitating issue that affects millions of people worldwide. Whether caused by muscle strains, herniated discs, or other underlying conditions, the impact of back pain on daily life can be significant.

In the pursuit of back pain relief, various medications play an essential role in managing and alleviating back pain symptoms.

Analgesics

Analgesics are pain-relieving medicines that are widely utilized to alleviate mild to moderate back pain.

Analgesics encompass various groups:

  • Paracetamol for mild pain
  • NSAIDs, including Ibuprofen, Diclofenac  and Naproxen
  • Cox-2 inhibitors like Celecoxib or Etoricoxib
  • Metamizole (Novalgin) for moderate to severe pain
  • Anapam (Nefopam) for moderate to severe pain
  • Pregabalin (Lyrica) or Gabapentin for neuropathic pain.
  • Opioids such as Tramal or Oxycodone for severe pain

Each subgroup targets pain via different mechanisms and potency levels.

They are also available in topical form as topical rubs and patches. While these topical preparations are typically used on joints like knees or hands, they can also be safe and beneficial for back pain.

Muscle Relaxants

Prescribed for muscle spasms alongside NSAIDs, common muscle relaxants like cyclobenzaprine, orphenadrine, and carisoprodol can aid in relieving back pain.

Muscle relaxants can be central and peripheral. Central muscle relaxants (e.g., cyclobenzaprine) depress the central nervous system, reducing muscle activity. Peripheral muscle relaxants (e.g., dantrolene) act directly on muscle fibers, disrupting muscle contraction.

Both types can help alleviate discomfort, but central muscle relaxants can cause drowsiness, dizziness, and confusion. It’s essential to consult with a healthcare provider to determine the most appropriate muscle relaxant and dosage based on individual needs and medical history.

Corticosteroids

This medicine for back pain are used to control inflammation.

In cases where systemic inflammatory conditions, like polymyalgia rheumatica or rheumatoid arthritis affecting the back, are responsible for inflammation, oral corticosteroids are recommended.

Alternatively, if the inflammation is confined to a specific joint in the spine, a corticosteroid injection directly into the inflamed joint may be administered.

To sooth the inflammation and swelling of the nerves, corticosteroids might be also administered close to the affected nerve either directly into the spinal canal (epidurally) or next to the nerve root exiting the spine (transforaminally)

Disease-Modifying Antirheumatic Drugs

Disease-modifying antirheumatic drugs (DMARDs) function gradually to alter the progression of inflammatory diseases.

Various DMARDs prove effective in managing multiple forms of arthritis impacting the spine, such as rheumatoid arthritis, ankylosing spondylitis, and psoriatic arthritis.

biologic response modifier

Biologic Response Modifiers

These are also disease-modifying medications, but unlike other DMARDs, they block steps in inflammation without suppressing the entire immune system.

They are approved to treat conditions like RA, PsA, ankylosing spondylitis, gout, lupus, juvenile arthritis, and non-radiographic axial spondylitis.
janus kinase inhibitors

Janus Kinase Inhibitors

Similar to biologics, these oral medications for back pain selectively target specific parts or pathways in the immune system rather than suppressing the entire system.

Janus kinase inhibitors (JAKi) are approved to treat psoriatic arthritis and psoriasis, rheumatoid arthritis, ankylosing spondylitis, juvenile arthritis, and nonradiographic axial spondyloarthritis.

Osteoporosis Medications

Osteoporosis medications aim to either slow bone loss or promote new bone formation.

While not directly treating back issues, these medications contribute to bone strength, reducing fracture risk. Bisphosphonates are also utilized in treating Paget’s disease of the bone, which can impact the spine.
